Indications:
Milam is used for short-term treatment of insomnia and for sedation before surgical or diagnostic procedures.
Description:
Milam (Midazolam) is a fast-acting sedative with a short duration of action. It has anxiolytic, anticonvulsant and muscle-relaxant properties.
Pharmacology:
Midazolam is a benzodiazepine that enhances the effect of GABA, a major inhibitory neurotransmitter in the central nervous system. This results in sedation, sleep induction, muscle relaxation, anxiolysis and amnesia.
Dosage and Administration:
Oral dose in adults is 7.5–15 mg daily; elderly patients should start with 7.5 mg. For premedication, 15 mg is given 30–60 minutes before procedures.
IV dose: 2.5 mg initially in adults (reduced to 1–1.5 mg in elderly or critically ill).
IM dose: 0.07–0.1 mg/kg in adults, lower in elderly; children 0.15–0.2 mg/kg.
Rectal dose (children): 0.35–0.45 mg/kg before anesthesia.
Drug Interactions:
Enhances sedative effects when used with antipsychotics, antidepressants, analgesics, anesthetics, antihistamines and other CNS depressants.
Contraindications:
Contraindicated in severe respiratory insufficiency, severe liver disease, myasthenia gravis, sleep apnea and hypersensitivity to benzodiazepines.
Side Effects:
Common effects include drowsiness, confusion, fatigue, headache and muscle weakness. IV/IM use may cause respiratory depression, apnea and changes in blood pressure or pulse.
Pregnancy and Lactation:
Avoid use during pregnancy unless absolutely necessary. Not recommended in breastfeeding as it passes into breast milk.
Precautions and Warnings:
IV administration should be given slowly. Monitor patients closely due to risk of respiratory depression.
Overdose Effects:
Overdose may cause coma, respiratory depression and apnea. It can be treated with flumazenil, a benzodiazepine antagonist.
