Febuxostat is a potent, non-purine selective inhibitor of xanthine oxidase, the enzyme responsible for the conversion of hypoxanthine to xanthine and subsequently to uric acid. It is primarily indicated for the chronic management of hyperuricemia in patients with gout. Unlike allopurinol, which is a purine analog, febuxostat does not interfere with other enzymes in the purine or pyrimidine metabolic pathways at therapeutic concentrations, offering a more targeted pharmacological profile. Reducing serum uric acid levels, it prevents the formation and deposition of monosodium urate crystals in joints and tissues, thereby decreasing the frequency of gout flares and the development of tophi. Clinically, febuxostat is often preferred for patients who do not tolerate allopurinol or have mild-to-moderate renal impairment, as it is primarily metabolized by the liver. However, during the initiation of therapy, it can paradoxically trigger gout attacks due to the mobilization of urate from tissue stores, necessitating co-administration with a prophylactic NSAID or colchicine. Practitioners should also monitor for cardiovascular events, as some clinical trials have suggested a higher risk of cardiovascular-related mortality in patients with pre-existing major heart conditions.
