Product Description

Indications:

Oramet is used to treat type 2 diabetes mellitus, especially in overweight patients when diet and exercise alone are insufficient. It can be used alone or with other antidiabetic drugs or insulin. It also helps reduce diabetes-related complications.

Pharmacology:

Metformin is a biguanide oral antidiabetic drug. It lowers blood glucose by reducing liver glucose production, decreasing intestinal absorption of glucose and improving insulin sensitivity. It does not usually cause hypoglycemia.

Dosage and Administration:

Adults: Start with 500 mg twice daily or 850 mg once daily with meals. Dose can be increased gradually up to 2000–2550 mg/day.

Children (≥10 years): Start with 500 mg twice daily, up to 2000 mg/day.

Extended-release: Start 500 mg once daily with evening meal, up to 2000 mg/day.

Do not crush XR tablets.

Drug Interactions:

May interact with drugs like nifedipine, cationic drugs (e.g., digoxin), and medications that increase blood sugar (e.g., corticosteroids, diuretics). Monitoring is required.

Contraindications:

Contraindicated in severe renal impairment, metabolic acidosis, liver disease, severe infection, dehydration, alcoholism and conditions causing low oxygen supply.

Side Effects:

Common: nausea, vomiting, diarrhea, abdominal pain, loss of appetite.

Rare: lactic acidosis, vitamin B12 deficiency, skin reactions.

Pregnancy and Lactation:

Insulin is preferred during pregnancy. Not recommended during breastfeeding due to limited safety data.

Precautions and Warnings:

Monitor kidney function regularly. Risk of lactic acidosis increases in renal impairment. May reduce vitamin B12 levels. Stop temporarily before surgery or contrast procedures.

Use in Special Populations:

Elderly patients require dose adjustment based on kidney function. Use cautiously in children and monitor growth.

Overdose Effects:

Overdose may cause lactic acidosis. Treatment includes supportive care and hemodialysis if needed.
