Metformin Hydrochloride is a biguanide antihyperglycemic agent that serves as the first-line gold standard for the management of Type 2 Diabetes Mellitus. Unlike sulfonylureas, metformin does not stimulate insulin secretion and therefore carries a minimal risk of hypoglycemia; instead, its primary mechanism of action involves the activation of AMP-activated protein kinase (AMPK). This activation leads to a significant reduction in hepatic gluconeogenesis—decreasing the amount of glucose produced by the liver—and enhances insulin sensitivity by increasing peripheral glucose uptake and utilization in skeletal muscles. Furthermore, metformin delays intestinal glucose absorption and has been noted for its "weight-neutral" or even weight-reducing effects, which is highly beneficial for diabetic patients struggling with obesity. Beyond its metabolic benefits, it is increasingly investigated for its cardioprotective properties and potential roles in managing Polycystic Ovary Syndrome (PCOS).
