Betahistine Dihydrochloride is an orally active histamine analogue used primarily for the symptomatic treatment of Ménière's disease and vestibular vertigo, characterized by episodes of dizziness, tinnitus, and progressive hearing loss. Its therapeutic efficacy is attributed to a dual mechanism of action: it acts as a strong antagonist of histamine H3 receptors and a weak agonist of histamine H1 receptors. By blocking presynaptic H3 receptors, it increases the release of neurotransmitters—most notably histamine—which promotes vasodilation in the microcirculation of the inner ear. This improved capillary permeability and blood flow in the stria vascularis help to reduce endolymphatic pressure (hydrops), addressing the underlying physiological cause of Ménière's symptoms. Additionally, Betahistine exerts a central effect by modulating the activity of the vestibular nuclei in the brainstem, thereby facilitating vestibular compensation and helping the nervous system adapt to balance disturbances. It is generally well-tolerated with a favorable safety profile, though it should be used with caution in patients with a history of peptic ulcer disease or bronchial asthma due to its histaminergic nature. Unlike traditional vestibular suppressants such as benzodiazepines or antihistamines, Betahistine does not typically cause significant sedation, making it a preferred long-term maintenance therapy for patients requiring consistent balance stabilization without impaired cognitive function.
