Indications: Spirocard Plus is indicated for essential hypertension, chronic congestive heart failure, hepatic cirrhosis with ascites, edema due to fluid retention, hyperaldosteronism, and resistant edema associated with secondary hyperaldosteronism.
Pharmacology: Spirocard Plus combines Spironolactone and Furosemide, which act through complementary mechanisms to produce enhanced diuretic effects. Furosemide is a loop diuretic that inhibits sodium, potassium, and chloride reabsorption in the ascending loop of Henle, increasing urinary excretion of water and electrolytes. Spironolactone is a potassium-sparing diuretic and aldosterone antagonist that reduces sodium reabsorption while conserving potassium in the distal renal tubules. Together, they promote effective diuresis while minimizing potassium loss.
Dosage & Administration:
Furosemide 20 mg + Spironolactone 50 mg: 1 to 4 tablets daily according to patient response.
Furosemide 40 mg + Spironolactone 50 mg: 1 to 2 tablets daily in patients requiring higher doses.
Not recommended for children. Elderly patients may require cautious dosing because of slower drug elimination.
Interaction: Concurrent use with ACE inhibitors or potassium supplements may increase the risk of hyperkalemia. Spironolactone may raise digoxin levels and increase risk of digitalis toxicity. Corticosteroids may enhance potassium loss. NSAIDs such as indomethacin may reduce the antihypertensive and diuretic effects of Furosemide. Furosemide may increase aminoglycoside-induced ototoxicity. Sucralfate may reduce absorption and effectiveness of Furosemide.
Contraindications: Contraindicated in anuria, acute renal insufficiency, severe renal impairment (creatinine clearance <30 ml/min), hyperkalemia, Addison’s disease, and hypersensitivity to spironolactone, furosemide, or sulfonamides.
Side Effects: Common adverse effects include headache, dizziness, drowsiness, gastrointestinal discomfort, cramps, diarrhea, skin rash, electrolyte imbalance, dehydration, hyponatremia, and hyperkalemia. Endocrine effects such as gynecomastia, menstrual irregularities, impotence, deepening of voice, and hirsutism may occur. Severe electrolyte imbalance may lead to cardiac arrhythmias.
Pregnancy & Lactation: Use during pregnancy only if the expected benefit outweighs potential fetal risk. Spironolactone metabolites and Furosemide are excreted in breast milk; breastfeeding should generally be avoided during therapy.
Precautions & Warnings: Use cautiously in patients prone to electrolyte imbalance, diabetes mellitus, enlarged prostate, hypotension, hypovolemia, and elderly patients. Regular monitoring of serum electrolytes and renal function is recommended.
