Merolin (Mirogabalin): Merolin is a gabapentinoid (γ-amino acid derivative) indicated for the treatment of neuropathic pain, including diabetic peripheral neuropathic pain (DPNP), postherpetic neuralgia (PHN), and peripheral neuropathic pain (PNP). It acts by selectively binding to the α2δ subunit of voltage-gated calcium channels (VGCCs), reducing calcium influx and the release of excitatory neurotransmitters, thereby decreasing neuronal hyperexcitability and relieving neuropathic pain. The recommended initial adult dose is 5 mg twice daily, gradually increased by 5 mg at intervals of at least one week to the usual maintenance dose of 10–15 mg twice daily, with dose adjustment required in patients with renal impairment. Merolin may interact with cimetidine, probenecid, lorazepam, and other OAT1, OAT3, OCT2, MATE1, MATE2-K, or UGT inhibitors, which may increase mirogabalin plasma concentrations, while alcohol should be avoided because it enhances dizziness and sedation. It is contraindicated in patients with hypersensitivity to mirogabalin. Common adverse effects include somnolence, dizziness, peripheral edema, weight gain, blurred or double vision, while less common but serious effects include loss of consciousness, liver dysfunction, jaundice, nausea, vomiting, and allergic reactions. It should be used with caution in patients with renal impairment, the elderly, and those operating vehicles or heavy machinery, as it may cause dizziness, drowsiness, and increase the risk of falls. During pregnancy and breastfeeding, it should be used only if the potential benefit outweighs the potential risk, as animal studies have shown placental transfer and excretion into milk. Merolin should be stored below 30°C, in a dry place, protected from light and moisture, and kept out of the reach of children.
