Rifagut (Rifaximin): Rifagut is a rifamycin-derived, non-systemic antibiotic indicated for the treatment of traveler's diarrhea caused by non-invasive Escherichia coli, reduction of recurrent overt hepatic encephalopathy, and bacterial overgrowth associated with irritable bowel syndrome (IBS). It works by inhibiting the β-subunit of bacterial DNA-dependent RNA polymerase, thereby blocking bacterial RNA synthesis and exerting a bactericidal effect against a broad range of Gram-positive, Gram-negative, aerobic, and anaerobic bacteria, with minimal systemic absorption. The recommended dose is 200 mg three times daily for 3 days for traveler's diarrhea (≥12 years), 550 mg twice daily for hepatic encephalopathy (≥18 years), and 400 mg three times daily for 10 days or 550 mg three times daily for 14 days for IBS with bacterial overgrowth; it may be taken with or without food. Rifagut has minimal drug interactions, although it may induce CYP3A4 in vitro; clinically significant interactions are uncommon at recommended doses in patients with normal liver function. It is contraindicated in patients with hypersensitivity to rifaximin, other rifamycin antibiotics, or any formulation component. Common adverse effects include flatulence, headache, abdominal pain, nausea, constipation, vomiting, fever, rectal tenesmus, and urgency of defecation, while rare but serious reactions include anaphylaxis, angioedema, exfoliative dermatitis, and pseudomembranous colitis. Rifagut should not be used for diarrhea accompanied by fever or bloody stools, and treatment should be discontinued if diarrhea worsens or persists for more than 24–48 hours, with alternative antibiotic therapy considered. It should be used with caution in patients with hepatic impairment because systemic exposure is increased; its use during pregnancy should be only if clearly needed, and breastfeeding requires caution as excretion into human milk is unknown. In overdose, supportive and symptomatic treatment is recommended. Rifagut should be stored below 30°C, protected from light and moisture, and kept out of the reach of children.
