Paricel (Rabeprazole): Paricel is a proton pump inhibitor (PPI) indicated for the treatment of duodenal ulcer, gastric ulcer, gastroesophageal reflux disease (GERD), Zollinger–Ellison syndrome, and Helicobacter pylori eradication (in combination therapy). It works by irreversibly inhibiting the gastric H⁺/K⁺-ATPase (proton pump), thereby suppressing gastric acid secretion. The dose depends on the indication and should be taken as prescribed by the physician. No dose adjustment is required in patients with renal or hepatic impairment, but it is not recommended in children because of insufficient safety and efficacy data. Common adverse effects include headache, diarrhea, constipation, nausea, vomiting, dizziness, rash, and dry mouth, while long-term use may increase the risk of osteoporosis-related fractures, hypomagnesemia, kidney problems, vitamin/mineral deficiencies, and subacute cutaneous lupus erythematosus (SCLE). Paricel should be used with caution during long-term therapy, and treatment should be discontinued if SCLE is suspected. It may also increase Chromogranin A (CgA) levels, potentially interfering with investigations for neuroendocrine tumors; therefore, it should be stopped at least 5 days before CgA testing (and repeat testing may be needed after 14 days if levels remain elevated). During pregnancy and breastfeeding, it should be used only if clearly needed and under medical advice. In overdose, symptoms are usually mild and reversible, with supportive and symptomatic treatment recommended; there is no specific antidote, and dialysis is ineffective because rabeprazole is highly protein-bound. Paricel should be stored below 30°C, protected from light and moisture, and kept out of the reach of children.
