## Indications & Pharmacology


* **Indications:** Treatment of nausea and vomiting of pregnancy in women who do not respond to conservative management.

* **Pharmacology:**

* *Doxylamine Succinate:* An antihistamine that crosses the blood-brain barrier to block $H_1$ receptors, inhibits the vestibular system, and reduces stimulation of the brain's vomiting center via muscarinic receptor inhibition.

* *Pyridoxine Hydrochloride:* A vitamin B6 analog with antiemetic properties.




## Dosage & Administration


* **Titration Schedule:**

* **Day 1:** Take one tablet orally at bedtime.

* **Day 2 (if symptoms persist):** Increase to two tablets daily—one in the morning and one at bedtime.



* **Maximum Dose:** Two tablets per day (one morning, one bedtime).

* **Instructions:** Take daily on an **empty stomach** with a glass of water. Swallow whole; do **not** crush, chew, or split. Do not take on an as-needed basis.


## Contraindications, Interactions & Warnings


* **Contraindications:** Hypersensitivity to doxylamine, ethanolamine derivatives, or pyridoxine; concurrent use with **monoamine oxidase inhibitors (MAOIs)**.

* **Drug Interactions:** Avoid concurrent use with alcohol, MAOIs (which intensify anticholinergic effects), and other CNS depressants (such as hypnotic sedatives and tranquilizers).

* **Precautions:** May cause significant somnolence (drowsiness). Patients must avoid driving or operating heavy machinery. Use with caution in women with asthma, glaucoma, peptic ulcers, or urinary obstruction.


## Side Effects, Pregnancy & Overdose


* **Side Effects:** Somnolence, sedation, and accidents related to CNS depression.

* **Pregnancy & Lactation:** Pregnancy **Category A**. Safe and specifically indicated for pregnancy-related nausea/vomiting. Both components are excreted in breast milk, so caution is advised during breastfeeding.

* **Overdose Effects:** Symptoms include restlessness, dry mouth, dilated pupils, sleepiness, vertigo, confusion, and tachycardia. Toxic doses can lead to seizures, rhabdomyolysis, acute renal failure, and death. Management includes gastric lavage, activated charcoal, and supportive care.
