Tizanidine hydrochloride is a centrally acting alpha-2 adrenergic receptor agonist primarily used as a skeletal muscle relaxant to manage spasticity associated with conditions such as multiple sclerosis, spinal cord injury, or stroke. By stimulating presynaptic alpha-2 receptors in the spinal cord, it reduces the release of excitatory amino acids, effectively inhibiting spinal interneurons and decreasing muscle tone without significantly affecting muscle strength. This mechanism makes it particularly useful for patients who need to maintain some motor function while reducing painful spasms. Pharmacokinetically, tizanidine has a short duration of action, necessitating frequent dosing to manage symptoms throughout the day. The most common side effects include dry mouth, somnolence, and hypotension; because of the latter, it must be used cautiously with other antihypertensives or potent CYP1A2 inhibitors, which can drastically increase its plasma concentration.
