Flunarizine hydrochloride is a selective calcium channel blocker with additional antihistaminic, antiserotonergic, and antidopaminergic properties, primarily indicated for the prophylaxis of classic and common migraine. Unlike calcium blockers used for hypertension, flunarizine is highly lipophilic and acts centrally by preventing cellular calcium overload in neurons during periods of brain ischemia or excessive stimulation. By inhibiting the transmembrane influx of calcium ions, it stabilizes neuronal excitability and reduces the frequency and severity of migraine attacks. It is also utilized in the management of vestibular disorders, such as vertigo and tinnitus, due to its vestibulocochlear suppressant effects. Because flunarizine has a very long biological half-life, its effects are cumulative, and it may lead to side effects like significant weight gain, drowsiness, and, in elderly patients, the potential development of extrapyramidal symptoms or depression.
