Entecavir is a potent guanosine nucleoside analogue used as a first-line antiviral therapy for the treatment of chronic Hepatitis B Virus (HBV) infection. It functions by selectively inhibiting the HBV polymerase through a multi-step mechanism, which includes blocking the priming of the viral polymerase, the reverse transcription of the negative strand from pre-genomic messenger RNA, and the synthesis of the positive strand of HBV DNA. By effectively suppressing viral replication, entecavir significantly reduces serum HBV DNA levels, promotes alanine aminotransferase (ALT) normalization, and improves histological liver outcomes, thereby lowering the long-term risk of cirrhosis and hepatocellular carcinoma. One of its most significant clinical advantages is its high genetic barrier to resistance in nucleoside-naive patients, making it a highly durable option for long-term maintenance.
