Folinic acid, also known as leucovorin, is a chemically reduced form of folic acid that plays a critical role in clinical pharmacology as a "rescue" agent and biochemical modulator. Unlike folic acid, it does not require the enzyme dihydrofolate reductase to be converted into its active form, allowing it to bypass the metabolic block created by folate antagonist drugs such as methotrexate. In oncology, it is extensively used in "leucovorin rescue" protocols to protect healthy cells from the lethal toxicity of high-dose methotrexate therapy while still allowing the chemotherapy to target malignant cells. Additionally, it is frequently administered in combination with 5-fluorouracil (5-FU) for the treatment of colorectal cancers, where it acts as a biochemical enhancer by stabilizing the binding of 5-FU to its target enzyme, thymidylate synthase, thereby increasing the drug's cytotoxic efficacy. Beyond its role in cancer treatment, folinic acid is also indicated for the prevention and treatment of folate deficiency where oral supplementation is insufficient and as a treatment for certain types of megaloblastic anemia.
