Indications:Indicated for the treatment of major depressive disorder (MDD).Pharmacology:Mirtazapine is an atypical antidepressant that enhances central noradrenergic and serotonergic activity. It blocks presynaptic α2-adrenergic receptors and antagonizes 5-HT2, 5-HT3, and histamine H1 receptors, contributing to its antidepressant and sedative effects. It is well absorbed orally and has a long half-life, allowing once-daily dosing.Dosage & Administration:The recommended starting dose is 15 mg once daily, preferably at bedtime. The usual effective dose range is 15–45 mg daily. Dose adjustments should be made at intervals of at least 1–2 weeks. Not recommended for use in children.Interaction:Significant interactions may occur with MAO inhibitors and other serotonergic drugs. Carbamazepine, phenytoin, and cimetidine may alter its effects. Alcohol and diazepam should be avoided during treatment.Contraindications:Contraindicated in patients with hypersensitivity to mirtazapine or its ingredients and in those using MAO inhibitors or within 14 days of MAOI therapy.Side Effects:Common side effects include drowsiness, dizziness, dry mouth, increased appetite, and weight gain.Pregnancy & Lactation:Pregnancy Category C. Use during pregnancy only if clearly needed. Patients should consult their physician if pregnant, planning pregnancy, or breastfeeding.Precautions & Warnings:Monitor for worsening depression, suicidal thoughts, behavioral changes, or mood disturbances, especially during treatment initiation. Use caution due to sedation and possible impairment of judgment or motor skills.
