Trimetazidine dihydrochloride is a metabolic agent classified as a selective 3-ketoacyl-CoA thiolase (3-KAT) inhibitor, primarily used in the long-term treatment of stable angina pectoris. Unlike traditional anti-anginal medications that focus on altering hemodynamics (such as heart rate or blood pressure), trimetazidine works at the cellular level by shifting myocardial energy metabolism from fatty acid oxidation to glucose oxidation. Because glucose oxidation requires less oxygen to produce the same amount of ATP, the drug helps maintain cellular energy levels and protects the heart from ischemia-induced injury. This "metabolic cytoprotection" helps preserve intracellular levels of phosphocreatine and prevents the accumulation of lactic acid, thereby improving exercise tolerance and reducing the frequency of anginal attacks. Although it is generally well-tolerated, it is typically contraindicated in patients with Parkinson’s disease or other movement disorders due to the risk of exacerbating extrapyramidal symptoms.
