Sodium alginate, sodium bicarbonate, and calcium carbonate is a fixed-dose combination commonly used as an antireflux agent to provide rapid relief from heartburn and acid indigestion. The primary mechanism is physical rather than purely chemical: upon contact with gastric acid, the sodium alginate reacts to form a thick, viscous alginate gel. Simultaneously, the sodium bicarbonate and calcium carbonate react with the acid to release carbon dioxide gas, which gets trapped within the gel, causing it to float on top of the stomach contents like a "raft. This physical barrier or "raft" effectively prevents the reflux of gastric acid and enzymes into the esophagus. Furthermore, the carbonates act as antacids by neutralizing excess stomach acid, while the calcium ions help cross-link and strengthen the alginate raft. Because this combination acts locally and is not absorbed into the bloodstream, it is often a preferred option for managing symptoms of Gastroesophageal Reflux Disease (GERD) in diverse patient populations, including during pregnancy.
