Indications:
Mirakof is used to relieve dry (non-productive) cough, including cough caused by viral infections. It is also used in pre- and post-operative cough, bronchoscopy procedures, whooping cough and cough associated with acute respiratory tract infections such as bronchitis, laryngitis and tracheitis.
Pharmacology:
Butamirate citrate acts directly on the brain’s cough center to suppress cough. It is non-opioid, non-sedating and not related to opium alkaloids. It is rapidly absorbed, highly protein-bound and has a half-life of about 13 hours. Its active metabolites also contribute to cough suppression.
Dosage and Administration:
Adults: 50 mg tablets 2–3 times daily or syrup 15 mL four times daily.
Adolescents (>12 years): 1–2 tablets daily or syrup 15 mL three times daily.
Children:
6–12 years: 10 mL syrup three times daily
3–6 years: 5 mL syrup three times daily
2 months–3 years: pediatric drops 0.5–0.75 mL four times daily
Drug Interactions:
Avoid use with expectorants, as it may cause mucus retention in the airways.
Contraindications:
Contraindicated in patients with hypersensitivity to butamirate citrate.
Side Effects:
Generally well tolerated. Rare side effects include rash, nausea, diarrhea and dizziness, which usually resolve after dose adjustment or discontinuation.
Pregnancy and Lactation:
Not recommended during the first trimester of pregnancy. Use with caution in later pregnancy. Use during breastfeeding only if benefits outweigh risks.
Precautions and Warnings:
Avoid combining with expectorants. If cough persists for more than 7 days (or more than 3 days in children), consult a doctor.
Overdose Effects:
Overdose may cause drowsiness, nausea, vomiting, diarrhea, loss of balance and low blood pressure. Management includes supportive treatment and emergency care.
