Sildenafil Citrate is a potent and selective inhibitor of phosphodiesterase type 5 (PDE5), an enzyme primarily found in the corpus cavernosum of the penis and the pulmonary vasculature. By inhibiting PDE5, it prevents the degradation of cyclic guanosine monophosphate (cGMP), which enhances the smooth muscle relaxant effects of nitric oxide and increases blood flow into the erectile tissue. Originally developed for hypertension, it is now most widely indicated for the treatment of erectile dysfunction (ED) and pulmonary arterial hypertension (PAH). Its pharmacological action is dependent on natural sexual stimulation to trigger the initial release of nitric oxide. While generally effective, it must be used with caution in patients with cardiovascular disease and is strictly contraindicated with the use of organic nitrates (such as nitroglycerin), as the combination can cause a life-threatening drop in blood pressure.
