Tadalafil is a potent, selective phosphodiesterase type 5 (PDE5) inhibitor primarily utilized in the treatment of erectile dysfunction (ED), benign prostatic hyperplasia (BPH), and pulmonary arterial hypertension. It works by inhibiting the PDE5 enzyme, which prevents the degradation of cyclic guanosine monophosphate (cGMP) in the smooth muscle cells of the corpus cavernosum and pulmonary vasculature. This leads to prolonged smooth muscle relaxation and increased blood flow in response to sexual stimulation. A distinguishing feature of tadalafil compared to other PDE5 inhibitors, such as sildenafil, is its significantly longer half-life (approximately 17.5 hours), which provides a therapeutic window of up to 36 hours, often earning it the moniker "the weekend pill." Clinically, it is contraindicated in patients using organic nitrates, as the combination can lead to a severe and potentially fatal drop in blood pressure. Common side effects include headache, dyspepsia, and back pain, which are generally mild and transient.
