Linagliptin is a highly selective, long-acting dipeptidyl peptidase-4 (DPP-4) inhibitor used in the management of type 2 diabetes mellitus to improve glycemic control by enhancing the body's natural incretin system. It works by inhibiting the enzyme responsible for the degradation of incretin hormones, such as glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP), which in turn stimulates insulin secretion and suppresses glucagon release in a glucose-dependent manner. A distinguishing pharmacological feature of Linagliptin compared to other DPP-4 inhibitors is its primarily non-renal route of excretion; it is largely eliminated unchanged via the enterohepatic system (feces), with less than 5% excreted through the kidneys. This unique pharmacokinetic profile makes it a preferred clinical choice for patients with varying degrees of renal impairment or hepatic insufficiency, as it typically requires no dosage adjustment regardless of the patient's kidney function. Clinical trials have demonstrated its efficacy in significantly reducing Hemoglobin A1c (HbA1c) levels with a low risk of hypoglycemia and a weight-neutral profile. While generally well-tolerated, clinicians should monitor for rare side effects such as nasopharyngitis or hypersensitivity reactions, and it is contraindicated in patients with a history of serious hypersensitivity to the drug or those with type 1 diabetes or ketoacidosis.
