Linezolid is a synthetic oxazolidinone antibiotic primarily used to treat serious infections caused by multidrug-resistant Gram-positive bacteria, including methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococcus (VRE). It exerts its bacteriostatic effect by binding to the 23S ribosomal RNA of the 50S subunit, effectively preventing the formation of the 70S initiation complex and halting bacterial protein synthesis. Because its mechanism of action occurs at an earlier stage than most other protein-synthesis inhibitors, cross-resistance with other antibiotic classes is rare. While it offers excellent oral bioavailability, clinical use requires monitoring for potential side effects such as myelosuppression (particularly thrombocytopenia) and interactions with serotonergic agents due to its weak, reversible inhibition of monoamine oxidase (MAO).
