Vildagliptin and metformin hydrochloride is a fixed-dose combination oral hypoglycemic therapy used to manage type 2 diabetes mellitus by targeting multiple pathophysiological pathways. Vildagliptin is a potent and selective dipeptidyl peptidase-4 (DPP-4) inhibitor that works by preventing the degradation of incretin hormones, such as GLP-1 and GIP; this enhances glucose-dependent insulin secretion from pancreatic beta cells while suppressing glucagon release from alpha cells. Metformin hydrochloride, a biguanide, complements this action by improving peripheral insulin sensitivity, decreasing intestinal glucose absorption, and inhibiting hepatic gluconeogenesis. Together, these agents provide superior glycemic control compared to monotherapy with a lower risk of hypoglycemia and weight gain.
