Nitrofurantoin is a unique synthetic nitrofuran antibiotic primarily used for the prevention and treatment of uncomplicated urinary tract infections (UTIs). Its mechanism of action is multifaceted and complex: once it is filtered into the urine and absorbed by bacteria, bacterial enzymes (nitroreductases) reduce the drug into highly reactive electrophilic intermediates. These intermediates then attack various bacterial targets simultaneously, including ribosomal proteins, DNA, RNA, and the enzymes involved in the citric acid cycle. This "multi-target" approach makes it very difficult for bacteria to develop resistance, which is why nitrofurantoin remains highly effective against common uropathogens like Escherichia coli and Enterococcus faecalis even after decades of use. Because it is rapidly excreted into the urine and achieves very low systemic concentrations, it is unsuitable for treating systemic infections or pyelonephritis. Patients are often advised to take it with food to enhance absorption and reduce gastrointestinal side effects, and they should be informed that it may harmlessly turn their urine a dark yellow or brown color.
