Indications:
This combination is used in essential hypertension, chronic congestive heart failure, hepatic cirrhosis with ascites, edema due to fluid retention, hyperaldosteronism and resistant edema associated with secondary hyperaldosteronism.
Pharmacology:
Furosemide is a loop diuretic that increases excretion of sodium, chloride and water, leading to fluid loss. Spironolactone is a potassium-sparing diuretic that blocks aldosterone, promoting sodium excretion while conserving potassium. Together, they provide a synergistic diuretic effect while reducing potassium loss.
Dosage and Administration:
Dose depends on patient condition and response.
Typical dose: 1–4 tablets daily (Furosemide 20–80 mg and Spironolactone 50–200 mg).
Higher strength tablets may be used in stabilized patients. Not recommended for children. Use with caution in elderly.
Drug Interactions:
Risk of hyperkalemia increases with ACE inhibitors or potassium supplements. May increase digoxin toxicity. NSAIDs may reduce effectiveness. Furosemide may increase ototoxicity with aminoglycosides.
Contraindications:
Contraindicated in anuria, severe renal impairment, hyperkalemia, Addison’s disease and hypersensitivity to the components.
Side Effects:
Possible effects include headache, dizziness, gastrointestinal upset, electrolyte imbalance (especially potassium changes), dehydration and hormonal effects such as gynecomastia and menstrual irregularities.
Pregnancy and Lactation:
Use only if benefits outweigh risks. Both drugs may pass into breast milk; breastfeeding is not recommended during treatment.
Precautions and Warnings:
Use cautiously in patients with electrolyte imbalance, diabetes, hypotension or prostate enlargement. Monitor fluid and electrolyte levels regularly.
