Hydroxyzine Hydrochloride is a first-generation antihistamine belonging to the piperazine class, distinguished by its potent H1-receptor antagonism and significant central nervous system activity. Unlike many newer antihistamines, it readily crosses the blood-brain barrier, providing effective relief not only for allergic conditions like pruritus and urticaria but also for the symptomatic management of anxiety and tension. It further exhibits antiemetic, sedative, and anticholinergic properties, making it useful as a pre-operative sedative to reduce anesthesia requirements. Its mechanism involves competing with histamine for receptor sites on effector cells in the gastrointestinal tract, blood vessels, and respiratory tract. While highly effective, its use is often limited by its sedative nature, and patients should be monitored for side effects such as drowsiness, xerostomia (dry mouth), and, in rare cases, QT interval prolongation.
