Indications: Demelon capsule is indicated for the symptomatic management of mild to moderately severe Alzheimer's disease dementia. It is also used for the treatment of mild to moderately severe dementia associated with Parkinson's disease.Pharmacology: Rivastigmine is a reversible cholinesterase inhibitor that inhibits both acetylcholinesterase and butyrylcholinesterase enzymes. This action prevents the breakdown of acetylcholine, increasing its availability in the brain and improving cholinergic function. It shows greater selectivity for central nervous system enzymes than peripheral ones.Dosage & Administration: Rivastigmine capsules should be taken orally twice daily with meals (morning and evening). The initial dose is 1.5 mg twice daily. If tolerated, the dose may be increased at intervals of at least 2 weeks to 3 mg, then 4.5 mg, and up to 6 mg twice daily. The effective maintenance dose ranges from 3 mg to 6 mg twice daily, with a maximum recommended dose of 6 mg twice daily. If treatment is interrupted for several days, therapy should be restarted at 1.5 mg twice daily and titrated again.Interaction: Rivastigmine may enhance the effects of succinylcholine-type muscle relaxants during anesthesia. It should not be used with other cholinergic drugs and may reduce the effects of anticholinergic medications.Contraindications: Contraindicated in patients with known hypersensitivity to rivastigmine or other carbamate derivatives.Side Effects: Common adverse effects include nausea, vomiting, diarrhea and weight loss, especially during dose titration. Other effects may include dizziness, bradycardia and gastrointestinal discomfort.Pregnancy & Lactation: Rivastigmine should be used during pregnancy only if clearly necessary. Breastfeeding is not recommended during treatment due to insufficient safety data.Precautions & Warnings: Use with caution in patients with cardiac conduction disorders, respiratory diseases such as asthma or COPD, peptic ulcer disease, urinary obstruction or seizure disorders. Regular monitoring of body weight is recommended.Overdose Effects: Symptoms of overdose may include nausea, vomiting, diarrhea, hypertension, hallucinations, bradycardia and syncope.
