Product Description

Indications: Evania is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It is also used to reduce the risk of cardiovascular death in adult patients with type 2 diabetes mellitus and established cardiovascular disease.

Pharmacology: Empagliflozin is a sodium-glucose co-transporter-2 (SGLT2) inhibitor that works by blocking glucose reabsorption in the renal tubules. This leads to increased urinary glucose excretion, reduction of blood glucose levels, and lowering of renal glucose threshold. It also contributes to modest weight loss and reduction in blood pressure.

Dosage & Administration: The recommended starting dose is 10 mg once daily, taken in the morning with or without food. If well tolerated, the dose may be increased to 25 mg once daily. Volume depletion should be corrected before initiating therapy.

Interaction: Co-administration with diuretics may increase urine output and risk of dehydration. Use with insulin or insulin secretagogues may increase the risk of hypoglycemia. Urine glucose tests and 1,5-anhydroglucitol (1,5-AG) assays are not reliable for monitoring glycemic control during therapy.

Contraindications: Contraindicated in patients with hypersensitivity to empagliflozin, severe renal impairment, end-stage renal disease or patients undergoing dialysis.

Side Effects: Common adverse effects include urinary tract infections and genital fungal infections. Other effects may include dehydration, hypotension, dizziness, weakness and increased thirst.

Pregnancy & Lactation: Use during pregnancy only if the potential benefit justifies the risk. It is not recommended during breastfeeding as safety data are insufficient.

Precautions & Warnings: Renal function should be assessed before starting treatment and monitored periodically. It should not be initiated in patients with eGFR below 45 ml/min/1.73 m². No dose adjustment is required for eGFR ≥45 ml/min/1.73 m².

Overdose Effects: In case of overdose, supportive treatment should be provided, including monitoring and removal of unabsorbed drug if necessary. The effectiveness of hemodialysis in removing empagliflozin is not established.
