Dexlansoprazole is a second-generation proton pump inhibitor (PPI) and the R-enantiomer of lansoprazole, primarily indicated for the treatment of erosive esophagitis and gastroesophageal reflux disease (GERD). Its clinical distinction lies in its unique Dual Delayed Release (DDR) formulation; unlike conventional PPIs that provide a single peak of drug concentration, Dexlansoprazole utilizes two different types of enteric-coated granules that release the medication at different pH levels in the small intestine. This results in two distinct peaks in plasma concentration—one occurring 1 to 2 hours after administration and a second between 4 to 5 hours—which extends the duration of gastric acid suppression and provides more consistent 24-hour symptom relief. By covalently binding to the H +/K+-ATPase enzyme system (the "proton pump") on the secretory surface of gastric parietal cells, it effectively inhibits the final step of acid production. Because its efficacy is less dependent on the timing of food intake compared to older PPIs, it offers greater dosing flexibility. However, long-term use requires monitoring for potential side effects such as vitamin B12 deficiency, hypomagnesemia, and an increased risk of bone fractures or Clostridioides difficile-associated diarrhea.
