Itopride Hydrochloride is a prokinetic benzamide derivative primarily utilized to treat functional dyspepsia and various gastrointestinal motility disorders. It functions through a dual mechanism of action: acting as a dopamine D2 receptor antagonist and an acetylcholinesterase inhibitor. By blocking these dopamine receptors, it relieves the inhibitory effect on gastrointestinal motility, while inhibiting the breakdown of acetylcholine increases its concentration at the muscarinic receptors of the smooth muscle. This combined effect enhances gastric accommodation, increases lower esophageal sphincter pressure, and accelerates gastric emptying. Clinically, it is effective in alleviating symptoms such as postprandial fullness, upper abdominal pain, anorexia, heartburn, and nausea. Unlike older prokinetics like cisapride, itopride has a high safety profile with minimal central nervous system penetration and no known cardiotoxicity (QT interval prolongation), as it is metabolized by flavin-containing monooxygenase 3 (FMO3) rather than the cytochrome P450 system. Common side effects are generally mild and may include headache, abdominal pain, or diarrhea.
