Ciprofibrate is a potent, third-generation derivative of fibric acid and a member of the fibrate class of lipid-regulating agents, primarily indicated for the treatment of severe hypertriglyceridemia and mixed hyperlipidemia. It acts as a selective agonist for the Peroxisome Proliferator-Activated Receptor-alpha (PPAR-Alpha), a nuclear receptor that regulates the expression of genes involved in lipid metabolism. By activating PPAR-alpha, ciprofibrate increases the activity of lipoprotein lipase, which accelerates the breakdown of triglyceride-rich lipoproteins (VLDL), and simultaneously enhances the synthesis of apolipoproteins A-I and A-II, leading to a significant rise in HDL ("good") cholesterol levels. It is characterized by an exceptionally long half-life of approximately 80 hours, which allows for convenient once-daily dosing but also requires careful consideration in patients with impaired renal function to avoid accumulation. While highly effective in reducing the risk of pancreatitis and cardiovascular events in high-risk patients, it must be used with caution when combined with statins due to an increased risk of myopathy and rhabdomyolysis. Regular monitoring of liver enzymes and renal function is typically recommended to ensure safety during long-term therapeutic management.
