Cildip, which contains cilnidipine, is a unique fourth-generation calcium channel blocker (CCB) used primarily for the management of hypertension. Unlike traditional CCBs (like amlodipine) that primarily block L-type calcium channels, cilnidipine is a dual L-type and N-type channel blocker. By inhibiting L-type channels in the vascular smooth muscle, it induces vasodilation and lowers blood pressure; meanwhile, its action on N-type channels—located at sympathetic nerve endings—inhibits the release of norepinephrine. This dual mechanism offers several clinical advantages, including a reduced risk of reflex tachycardia (increased heart rate) and a lower incidence of pedal edema (ankle swelling) compared to older CCBs. Furthermore, cilnidipine is noted for its renoprotective effects, as it helps dilate both afferent and efferent arterioles in the kidney, which reduces glomerular pressure. It is typically administered once daily and is particularly beneficial for hypertensive patients with co-existing conditions like chronic kidney disease or diabetes. Typical side effects are mild and may include dizziness, headache, or flushing.
