Olmesan mainly contains Olmesartan medoxomil, a potent, long-acting angiotensin II receptor blocker (ARB) used primarily for the management of hypertension. It is a prodrug that is rapidly converted during gastrointestinal absorption to its active form, olmesartan, which then selectively blocks the binding of angiotensin II to the AT1 receptor in vascular smooth muscle and the adrenal gland. Inhibiting this pathway, it prevents vasoconstriction and the release of aldosterone, leading to decreased systemic vascular resistance and reduced blood pressure. Olmesartan is noted for its high affinity and slow dissociation from the receptor, providing sustained 24-hour efficacy with once-daily dosing. While it is generally well-tolerated, rare but serious side effects include sprue-like enteropathy (severe chronic diarrhea and weight loss) and potential hyperkalemia, particularly in patients with renal impairment.
