Clopid contains Clopidogrel bisulfate, a potent oral antiplatelet agent belonging to the thienopyridine class, widely prescribed to reduce the risk of myocardial infarction, stroke, and vascular death in patients with atherosclerosis or those undergoing coronary stenting. It functions as a prodrug, requiring hepatic bioactivation via the cytochrome P450 system—specifically the CYP2C19 enzyme—to form its active metabolite. This active form irreversibly inhibits the P2Y12 receptor on the platelet surface, preventing the binding of adenosine diphosphate (ADP) and subsequent activation of the glycoprotein IIb/IIIa complex, which effectively inhibits platelet aggregation for the remainder of their lifespan. While highly effective, clinical management must account for "clopidogrel resistance" in poor metabolizers and the increased risk of gastrointestinal bleeding, especially when used in dual antiplatelet therapy (DAPT) with aspirin.
