Telma contains Telmisartan, a potent, long-acting angiotensin II receptor blocker (ARB) primarily used in the management of hypertension and for the reduction of cardiovascular risk in high-risk patients. It works by selectively antagonizing the AT-1 receptor, which prevents the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to systemic vasodilation and reduced blood pressure. A unique pharmacological feature of telmisartan is its partial agonism of the peroxisome proliferator-activated receptor gamma, which may provide additional metabolic benefits by influencing glucose and lipid metabolism. Due to its high lipophilicity and exceptionally long terminal elimination half-life of approximately 24 hours, it offers sustained blood pressure control throughout a once-daily dosing interval. It is generally well-tolerated, though clinical monitoring for hyperkalemia and renal function is standard practice, particularly when used alongside other medications affecting the renin-angiotensin-aldosterone system.
