Tranexamic acid (TXA) is a synthetic derivative of the amino acid lysine that functions as a potent antifibrinolytic agent. It works by reversibly binding to the lysine-binding sites on plasminogen molecules, which effectively prevents their activation into plasmin. Since plasmin is the primary enzyme responsible for degrading fibrin clots, TXA helps stabilize existing blood clots and reduces excessive bleeding. Clinically, it is widely utilized in both emergency and elective settings, ranging from the management of heavy menstrual bleeding and tooth extractions in hemophilia patients to major surgical procedures and traumatic injury cases (such as those addressed in the CRASH-2 trial). While generally well-tolerated, its use requires caution in patients with a history of thromboembolic disease due to the theoretical risk of promoting unwanted clot formation.
