Ticagrelor is an orally administered, direct-acting antiplatelet agent belonging to the novel chemical class of cyclopentyltriazolopyrimidines. Unlike thienopyridines such as clopidogrel and prasugrel, ticagrelor is not a prodrug and does not require hepatic metabolic activation to exert its effects. Its mechanism involves the reversible and non-competitive antagonism of the P2Y12 receptor on the platelet surface, where it binds to a site distinct from the endogenous agonist adenosine diphosphate (ADP). By blocking this receptor, ticagrelor inhibits ADP-mediated platelet activation and aggregation, thereby reducing the risk of atherothrombotic events. Clinically, it is indicated for patients with Acute Coronary Syndrome (ACS), a history of Myocardial Infarction (MI), or high-risk Coronary Artery Disease (CAD) to decrease the incidence of cardiovascular death, MI, and stroke. A unique aspect of its pharmacology is its ability to inhibit the equilibrative nucleoside transporter-1 (ENT1), which increases local extracellular adenosine levels, potentially contributing to its cardioprotective effects but also its distinct side effect of dyspnea (shortness of breath). Other significant adverse reactions include an increased risk of bleeding, hyperuricemia (which may trigger gout), and transient ventricular pauses. It is primarily metabolized by CYP3A4, making it susceptible to drug interactions with strong inhibitors or inducers of this enzyme. Ticagrelor is contraindicated in patients with active pathological bleeding, a history of intracranial hemorrhage, or severe hepatic impairment.
