Clopidogrel bisulphate is a potent oral thienopyridine antiplatelet agent primarily used to reduce the risk of atherosclerotic events, such as myocardial infarction, stroke, and vascular death. As a prodrug, it requires hepatic bioactivation—largely through the CYP2C19 enzyme—to form its active metabolite, which irreversibly binds to the P2Y12 purinergic receptors on the surface of platelets. This binding inhibits the activation of the glycoprotein IIb/IIIa complex, thereby preventing adenosine diphosphate (ADP)-induced platelet aggregation for the remainder of the platelet's lifespan (approximately 7 to 10 days). It is frequently prescribed for patients with acute coronary syndrome (ACS) or those undergoing percutaneous coronary intervention (PCI), often as part of a dual antiplatelet therapy (DAPT) with aspirin. Clinicians must exercise caution when prescribing clopidogrel alongside proton pump inhibitors like omeprazole, as they can inhibit the CYP2C19 enzyme and diminish the drug's antiplatelet efficacy.
