Dienogest is a unique fourth-generation progestin that combines the pharmacological properties of 19-nortestosterone and progesterone derivatives. It is primarily indicated for the treatment of endometriosis, as well as for use in combined oral contraceptives. Unlike many other progestins, dienogest lacks androgenic activity and instead exhibits significant anti-androgenic effects, making it effective for managing symptoms like acne or hirsutism. Its mechanism of action in endometriosis involves the suppression of trophic effects of estradiol on ectopic endometrial tissue, as well as a reduction in the production of inflammatory cytokines. Creating a hypoestrogenic and hyperprogestogenic local environment, it leads to the involution of endometriotic lesions and a significant reduction in pelvic pain. While highly effective, common side effects may include irregular menstrual bleeding (spotting), headaches, and breast tenderness.
