Testosterone Undecanoate is a long-acting ester of the natural androgen testosterone, primarily utilized in hormone replacement therapy (HRT) for the treatment of primary and secondary hypogonadism in men. Due to the addition of the fatty acid undecanoate ester, the medication is highly lipophilic, allowing it to be administered either orally in oil-filled capsules—which are absorbed via the lymphatic system to bypass first-pass hepatic metabolism—or as an extremely long-lasting intramuscular injection. The injectable formulation is particularly noted for its steady-state pharmacokinetic profile, often requiring dosing only once every 10 to 14 weeks, which minimizes the "peak and valley" fluctuations in mood and energy levels commonly seen with shorter-acting esters. It plays a critical role in restoring physiological testosterone levels, thereby improving bone density, muscle mass, libido, and secondary sexual characteristics.
