Dydrogesterone is a potent, orally active synthetic progestogen that is molecularly closely related to natural progesterone, categorized as a "retroprogesterone" due to its unique spatial configuration. Unlike many other synthetic progestins, it lacks androgenic, estrogenic, or glucocorticoid properties, making it highly selective for progesterone receptors. Its primary therapeutic applications include the treatment of progesterone deficiencies such as endometriosis, secondary amenorrhea, irregular cycles, and premenstrual syndrome. Crucially, it is widely utilized in reproductive medicine for luteal phase support during assisted reproductive technology (ART) and for reducing the risk of miscarriage in patients with threatened or recurrent pregnancy loss. Because it does not inhibit ovulation at standard therapeutic doses, it allows for cycle regulation without preventing conception.
