Phenoxymethylpenicillin, commonly known as Penicillin V, is an orally active, narrow-spectrum beta-lactam antibiotic primarily used to treat mild-to-moderate infections caused by susceptible Gram-positive organisms, such as Streptococcus pyogenes. It functions by inhibiting bacterial cell wall synthesis through the binding of penicillin-binding proteins (PBPs), which disrupts the cross-linking of peptidoglycan and leads to bacterial lysis. Unlike Penicillin G, Phenoxymethylpenicillin is acid-stable, allowing it to resist degradation by gastric acid and making it suitable for oral administration. It remains the drug of choice for conditions such as streptococcal pharyngitis (strep throat) and for prophylaxis in patients with rheumatic fever. However, its clinical utility is limited against Gram-negative bacteria and beta-lactamase-producing strains, and it is contraindicated in individuals with a known penicillin hypersensitivity.
