Indications: Linez is indicated for the treatment of various Gram-positive infections including nosocomial pneumonia, community-acquired pneumonia, complicated skin and soft tissue infections (including diabetic foot infections without osteomyelitis), uncomplicated skin and soft tissue infections, and infections caused by vancomycin-resistant Enterococcus faecium. It is not indicated for Gram-negative infections. Safety and efficacy beyond 28 days of therapy have not been established.
Pharmacology: Linezolid is a synthetic antibiotic belonging to the oxazolidinone class, active mainly against Gram-positive bacteria. It inhibits bacterial protein synthesis by binding to the 23S rRNA of the 50S ribosomal subunit, preventing formation of the functional 70S initiation complex required for bacterial translation. It is generally bacteriostatic against enterococci and staphylococci, but bactericidal against most streptococci.
Dosage & Administration: Linezolid may be administered orally with or without food or via intravenous infusion over 30–120 minutes. For adults and adolescents (≥12 years): 600 mg every 12 hours for most infections (duration 10–14 days for pneumonia, up to 14–28 days for complicated infections). For uncomplicated skin infections: adults 400 mg every 12 hours, adolescents 600 mg every 12 hours for 10–14 days. For pediatric patients (birth to 11 years): 10 mg/kg every 8 hours (or every 12 hours in specific age groups). Neonates may require adjusted dosing based on age and response. No dose adjustment is required when switching from IV to oral therapy.
Interaction: Linez is a reversible, non-selective monoamine oxidase (MAO) inhibitor and may interact with adrenergic and serotonergic drugs. Concomitant use with SSRIs, TCAs, triptans, meperidine, buspirone, or sympathomimetics may increase the risk of serotonin syndrome or hypertensive reactions. Patients should avoid foods rich in tyramine (e.g., aged cheese, fermented products) during therapy.
Contraindications: Contraindicated in patients with hypersensitivity to linezolid or its components. It should not be used with MAO inhibitors (e.g., phenelzine, isocarboxazid) or within 14 days of such therapy. Also contraindicated in patients with uncontrolled hypertension, pheochromocytoma, thyrotoxicosis, carcinoid syndrome, or those receiving certain serotonergic or adrenergic drugs.
Side Effects: Common side effects include diarrhea, headache, and nausea. Other adverse effects may include oral or vaginal candidiasis, hypertension, dyspepsia, abdominal pain, itching, and tongue discoloration. Most adverse reactions are mild to moderate.
Pregnancy & Lactation: Available data do not indicate a clear risk of major birth defects; however, use during pregnancy should be based on benefit-risk assessment. Linezolid is excreted in breast milk; therefore, breastfeeding infants should be monitored for gastrointestinal disturbances such as diarrhea or vomiting.
Precautions & Warnings: Weekly monitoring of complete blood count is recommended, especially during prolonged therapy (>2 weeks) or in patients with bone marrow suppression. Visual disturbances require immediate evaluation. There is a risk of Clostridium difficile-associated diarrhea even weeks after therapy. Use cautiously in patients with hypertension or those on interacting drugs. Monitor for lactic acidosis, hypoglycemia, and signs of serotonin syndrome.
Overdose Effects: No specific overdose cases reported. Management includes supportive care and monitoring. Approximately 30% of the drug may be removed by hemodialysis over 3 hours.
Therapeutic Class: Oxazolidinone antibiotic
Reconstitution: For oral suspension, add 75 ml of boiled and cooled water to the bottle in two portions and shake well after each addition until fully mixed. Shake before each use and use within 21 days. IV formulation is supplied ready-to-use and should not be mixed with additives or used in series connections.
Storage Conditions: Store at room temperature (15–30°C), protected from light and moisture. Keep out of reach of children.
