Ivabradine is a unique cardiotonic agent that acts as a pure heart-rate-lowering drug by selectively and specifically inhibiting the hyperpolarization-activated cyclic nucleotide-gated (HCN) channels, often referred to as the "funny" (If) current, within the sinoatrial node. Unlike beta-blockers or calcium channel blockers, Ivabradine reduces the spontaneous pacemaker activity of the SA node without affecting myocardial contractility (inotropy), ventricular repolarization, or systemic blood pressure. This allows for a reduction in heart rate that effectively increases diastolic perfusion time and reduces myocardial oxygen demand, making it particularly valuable in managing chronic stable angina pectoris and heart failure with reduced ejection fraction (HFrEF) in patients who are in sinus rhythm. Because the If current is also present in the retina (the Ih current), a common and distinct side effect is phosphenes, described as transient enhanced brightness in limited areas of the visual field. Clinical administration requires careful monitoring for bradycardia and is contraindicated in patients with a resting heart rate below 60 beats per minute, sick sinus syndrome, or third-degree heart block.
