Terbinafine hydrochloride is a highly lipophilic allylamine antifungal agent widely used for the treatment of dermatophyte infections involving the skin and nails, such as onychomycosis, tinea pedis, and tinea corporis. It functions by inhibiting the enzyme squalene epoxidase, a key step in the fungal ergosterol biosynthesis pathway. This inhibition leads to a dual effect: the deficiency of ergosterol weakens the fungal cell membrane, while the intracellular accumulation of squalene exerts a fungicidal effect. Terbinafine is particularly valued for its ability to concentrate in the stratum corneum, sebum, and hair follicles, allowing for sustained therapeutic levels long after the treatment course is completed. While generally effective and well-tolerated, its oral administration requires monitoring for potential hepatotoxicity and may rarely cause taste disturbances (ageusia).
