Nitroglycerin is a potent vasodilator primarily used in the management and prevention of angina pectoris (chest pain) associated with coronary artery disease. As a pro-drug, it is converted within the body into nitric oxide (NO), which activates the enzyme guanylate cyclase, leading to increased levels of cyclic guanosine monophosphate (cGMP). This biochemical cascade results in the relaxation of vascular smooth muscle, predominantly in the venous system, which reduces cardiac preload and myocardial oxygen demand. It is typically administered via sublingual tablets or sprays for rapid relief of acute attacks due to its extensive first-pass metabolism when swallowed, though transdermal patches are also used for long-term prophylaxis. Common side effects include throbbing headaches, hypotension, and reflex tachycardia, and it is strictly contraindicated for use alongside phosphodiesterase-5 (PDE5) inhibitors.
