Prelica, which contains pregabalin, is a medication classified as a gabapentinoid, primarily used to treat neuropathic pain, epilepsy, and generalized anxiety disorder. Although structurally related to the neurotransmitter gamma-aminobutyric acid (GABA), it does not bind directly to GABA receptors; instead, it binds to the alpha2_delta subunit of voltage-gated calcium channels in the central nervous system. This action reduces the influx of calcium into nerve terminals, thereby modulating the release of several excitatory neurotransmitters, such as glutamate and substance P, which helps calm overexcited neurons. Clinically, it is highly effective for managing chronic pain conditions like postherpetic neuralgia, diabetic peripheral neuropathy, and fibromyalgia. It is also utilized as an adjunctive therapy for adult patients with partial-onset seizures. Pregabalin is characterized by rapid absorption and high bioavailability, but because it is predominantly excreted unchanged by the kidneys, dosage adjustments are necessary for patients with renal impairment. Common side effects include dizziness, somnolence, peripheral edema, and weight gain, and it is generally advised to taper the dose gradually when discontinuing treatment to avoid withdrawal symptoms.
