Calcitriol is the hormonally active form of vitamin D, chemically known as 1,25-dihydroxyvitamin D3 (1,25(OH)2D3 ). It is synthesized in the kidneys from its precursor, calcifediol, through the enzymatic action of 1-alpha-hydroxylase. Calcitriol plays a critical role in calcium and phosphate homeostasis by binding to the vitamin D receptor (VDR) in the intestines to promote dietary calcium absorption and in the kidneys to reduce calcium excretion. It also acts directly on bone tissue to regulate mineralization and resorption. Clinically, it is used to treat secondary hyperparathyroidism, hypocalcemia in patients undergoing chronic renal dialysis, and metabolic bone diseases. Because it is the active form of the vitamin, it does not require metabolic activation by the liver or kidneys, making it particularly effective for patients with severe renal impairment, though it requires careful monitoring to prevent hypercalcemia.
