Folinic acid, also known as leucovorin, is a 5-formyl derivative of tetrahydrofolic acid that serves as a vital biochemical cofactor in nucleic acid synthesis. Unlike folic acid, it does not require the enzyme dihydrofolate reductase (DHFR) for conversion, allowing it to bypass the metabolic blockade created by folate antagonists. In clinical practice, it is primarily used in "leucovorin rescue" protocols to reduce the systemic toxicity of high-dose methotrexate by providing a source of functional folate to healthy cells. Additionally, it is frequently co-administered with 5-fluorouracil (5-FU) in colorectal cancer treatments, where it acts as a biochemical modulator to enhance the binding of 5-FU to thymidylate synthase, thereby increasing the drug's cytotoxic efficacy. Beyond oncology, folinic acid is also employed to treat megaloblastic anemia resulting from folate deficiency and to manage toxicities associated with DHFR inhibitors like trimethoprim or pyrimethamine.
