Mirogabalin besylate is a novel ligand for the alpha-2 delta subunit of voltage-gated calcium channels, specifically designed for the treatment of peripheral neuropathic pain, such as diabetic peripheral neuropathic pain (DPNP) and postherpetic neuralgia (PHN). Developed as a more selective alternative to earlier gabapentinoids like pregabalin and gabapentin, it exhibits a slower dissociation rate from the alpha-2 delta-1 subunit—which is associated with analgesic effects—compared to the alpha-2 delta-2 subunit, which is linked to central nervous system side effects like somnolence and dizziness. By binding to these subunits, mirogabalin reduces the influx of calcium into hyperexcitable neurons, thereby inhibiting the release of excitatory neurotransmitters in the spinal cord and providing sustained pain relief.
